need help with smoke effect

As IamSam has already explain, its done with a brush, you will have to practice with a smoke/mist type brush , then try opacity levels, if you have a tablet use the brush settings, adjust the flow levels, so on and so on, eventually you will get what your looking for.

Search Google for free to use smoke or mist brushes , Also try Deviant Art, there are some good brushes there that people let you have for free.

Try things out and good luck.

I think he's using a cloud brush.

Take a look at 2:05 of the video. He chooses what look like a cloud brush.

I could not find the exact cloud brush he used but I just did a search for Photoshop Transparent Cloud Brushes.

Using the settings from the video I made this with a cloud brush. It's soft, misty, and like fog or smoke.
