How can I make this effect on PS?

Hi Awoi and welcome to PSG.
I'm certainly a fan of glitch art but I have to admit that I have not created any myself. When I read this thread, I began looking for tutorials. After searching, I found these two tutorials that I would use as a starting point for re-creating the image in your example.
The first would be a variation of the this tutorial to create the digitized/pixelated effect:

Then this tutorial for for the rest:

Techniques in the second tutorial could also be applied the the first as well.
It's a matter of experimentation.
