    My first observation is that the layers are out of order. They should appear like this.... This explains why the Black layer was set to Multiply and I would lower it's opacity. It also allows for the shadowing of the right paper sheet.

    Create a new layer. Select the Type Tool. Select the font, font size, and font color. Click on the canvas where you want the filename to be. Type out the filename. You can reposition with the Move Tool if needed. Save as a jpg.
