    How has he done this ?

    This is one of those things that looks more difficult than it really is, if I was to do this same picture to look as realistic as posible I'll just prop the "dice" on a table and take pictures and use photoshop to add them in. Some things are better done this way.

    I did a rolling dice over a table a while back ago and used the 3D features of PS CC and a bit of tinkering I got a good look.

    I believe it has to do with the way the 3D text is projecting the shadows, try moving the shadows around and see if that helps. Also, what do you mean by gray background? just change the color to gray?

    I'm sure when printing you can select the color of ink you want, otherwise layer one can be changed to any color. Same as the G and B on photoshop. Fairly simple.

    This is a different view... the eyes are not perfectly equal but it gives it a more realistic feel.
