Creating a layered file so I can insert my image over & over for pinback buttons

I'd like to be able to make all my auctions look like this one:

Prince William Kate Middleton Engagement British by CreativeClam

How do I do this? I'm assuming it would be an editable multi-layered PSD file but I don't know how to do the part that makes it LOOK like a mylar. I can do a LOT in Photoshop but there is a LOT I can't do! Thanks so much!

I should add: Creating pins in Illustrator | Veerle's blog is exactly what I'm trying to do so that I can retain the layers and insert a different image every time. I had never used my Illustrator before and was getting hung up- this girl said she would post the tutorial for doing the same thing in Photoshop but never did- does anyone know how to do this in Photoshop? Thanks!
