Quick Mask not working CS4

My quick mask mode was working until a few weeks ago. I click on quick mask to cut out a section of a photo, then I use the brush tool. I cut out the section, I de-select the mask tool and then i make a new layer copy of that section. but everytime I do this, it seems to look transparent; like its not cutting the whole thing. my opacity is set to 100%, flow is to 100%, hardness set to 100%, but it will not cut out its whole opacity. I don't know what to do!!

Check the icon I highlighted on the top tool bar. If clicked pen tablet pressure will overide the brush panel opacity settings.

My photoshop is on Mac and does not look like that. where can I find that icon because it is not located on that screen?
