what effect are you referring to?
there is a few things on that image that i would class as different effects.
but if your referring to the crinkled paper then just scan i bit of crinkled paper/cardbaord

The paper you can download lots out there, or scan your own maybe.
The face is probably done with threshold then a paint effect maybe, all in Photoshop.

I use cs5 and cs2 sometimes, there all standard filters within the suite, the downloads are from places like 40+ Various High Quality Free Paper Patterns and Textures - You The Designer

when he says download he means find an image of paper on the net.
this site is good for images they have a whole category for paper
[CG Textures] - Textures for 3D, graphic design and Photoshop!
