    Illustrator How was this done?

    In Adobe Illustrator after you have made the states, click on one and choose Effect --> 3D --> Extrude and Bevel from there you can set the amount and angle of the extrusion.

    Distortion map question

    Not that I know of, but if you distort/stretch the first image in Photoshop then maybe there is a way to apply the same effects to a second image.... just depends on what you are doing to it. You could either record an action or copy over duplicate adjustment layers and layer effects.
