    Pretty close, Welles! The only reason I think it is a stock PS filter, and not a tutorial effect is the opposite direction strokes most visible in the trees. That's almost a dead PS filter giveaway.

    How can I re-create this text effect?

    You'll have to do a web search for the tuts, just type in Photoshop 3d text tutorial. The effect is a 2-3 layer 3d text tutorial, extended 90 degrees to the right. Then the original text layer is given a light drop shadow. This effect will work best with letters with sharp edges.

    First change Image>Mode to RGB if it is grayscale. Then Image>Adjustments>Hue/Saturation, check the colorize box, slide the Brightness slider to about +15 to 20, move the saturation slider to about +10, and adjust hue to the color you want, moving the sliders as needed for depth and brightness.

    Have you tried using a search engine with the keywords "CD case cover tutorial"??? There are about a half dozen tuts out there, some with templates. As for the design itself, there are no "talent/skill" tutorials, so you'll have to do that yourself. :\
