    I noticed the noise in the background particularly after the B&W conversion. 'Tis why I added back in some noise after smoothing. I tried a different approach that adds in some gaussian blur in the background and no noise added. I did bump the levels a tick.

    Are you applying the style to a layer that encompasses the entire canvas? If so, the effects may be outside the canvas area. You should also check Layer > Layer Style > Show Effects is selected (should show up in menu tree as Hide All Effects if the Show Effects is enabled).

    I agree with Paul. I cannot for the life of me figure out why anyone would add such a heavy handed color overlay that yellows the teeth of the subjects. This is generally an undesired side effect. I used the Auto Color function and found the original color palette to be more palatable.
