Cannot Reset Preferences

I'm having weird problems with my toolbar in CS5 and I've been trying to reset preferences. I hold down Shift+Ctrl+Alt and launch Photoshop, but Photoshop just launches normally and I do not get a window asking me to delete preferences. Does anybody know of another way to delete preferences?

While I'm at it, here is the problem I'm having: Whenever I click on a tool with either the mouse or the tablet stylus, Photoshop somehow interprets it as a right-click. Instead of simply selecting the tool I want, Photoshop opens the little fly-out window where, for example, you can switch from the brush tool to the pencil tool. If I right-click my mouse on a tool, the same thing happens, so it's not as if I accidentally reset my mouse controls.

Choose Run as Adminstrator, and then do the Ctrl+Alt+Shift thing? Maybe that could help...
 
Update... even though I didn't get a pop-up window asking me to reset preferences, they seem to have been reset anyway. Various other settings that I had previously chosen have been reset to their defaults. But the original problem is still not solved. Whenever I select a tool I still get the little fly-out window.

I'm now starting to suspect a problem with my mouse, but the same thing happens when I use my Wacom tablet, so it's hard to believe two different devices failed simultaneously. I may need to start drinking heavily and hope the problem goes away by itself.
