BTW, if you are interested in following Sam's first suggestion, ie, generating the effect in-camera using a prism optical filter, here's a link that illustrates the effects that are available with this technique:

Take a few exposures, rotating the prism filter 10 or 20 degrees between each shot and blend them together and you will be able to get very close to the example you posted.


That being said, hardly any photographers use such optical filters any more since it is so easy to superimpose multiple images of different sizes and shapes using photoshop's native tools.
