I found this stumbling around the web, can anyone tell how how it looks so realistic, or perhaps find a tutorial?

I think you can still do something Similar this. Use a picture of someone and layer some pictures you'd like to place on their body. Use fade button so body shows though. use paint too. Like for his face, turn it to blue cut out and put over original, The Hot pinkish part same thing but turn to pink, then over lay some kind of grunge layer.
 
You can several ways of doing it depending on the boldness of the colour

Step 1 select the person you want to colour and put them on there own seperate layer

convert to black and white and delete the background layer so all you have is the person you want to bidypaint digitally save that as a psd.

Now undo the black and white and the deletion of the background layer.

Create a new layer and draw your shape on it and fill with colour 1 layer per colour then add a stroke to it in blending options.

Go to filter > Distort > Displace keep default settings at first tweak them afterwards. then click ok it will ask you to load a file select the black and white psd you saved earlier.
You will see that will deform your shape layer to make it fit better within the body now just set the layer blending mode to overlay, colour or softlight whichever you prefer. right click the layer and select create clipping mask so it will only effect the selection of your model (the better selection you make the better the results.

If you want to skip the displacement stage you can In my example the magenta\\ pink is with displacement and the green is just a new layer set to overlay and the colour shape. This way you dont have to save a black and white version but still best to extract your model from the background so you can clip it.

I will also point out the skin tone makes how impressive this effect is the closer the skin is to pure white or pure black then the colours will not work so you need to have a nice tanned colour for best results.
 
I think the pink with the displacement map works the best. It seems to have just a slight bit better appearance of real skin. Of course I can't compare it to the original.
 
Well obviously they are standard displacement settings they are to high in my example so if you tone it down to about 5 you will get a better result.
