As any explanation I give will not be understood, I have done an Illustrator file for you to look at, to see how it is made. I made it look slightly different for legal reasons, but it is all there for you to see. This is what it looks like:-

I did it in Illustrator, as that is the kind of program that is used to make these kind of images.
