Print wrong size

I am having a new problem with prints. Set CS5 to print 8.5x11 for a test print. When I set it to 11x14 for the final print, I keep getting an 8.5x11 on the 11x14 paper. Set print settings to 11x14, the preview picture is 11.389x14.389 and the picture is full size. Scaled to print size shows 11.305x14.388 and I tried "scale to fit media" checked and unchecked with no difference. I have printed 11x14 before with no problems. This was before I updated to Lightroom 4 which is the only change on this computer. I updated CS5 to latest addition and repaired disc permission but still have the problem. Mac OS x 10.6.8. I have run out of ideas!!!
