I know what you mean, in that case what i used to do is set the output to new layer whit layer mask and then paint back manually those small details in the image.

Also when in refine edge window don't go straight for the smart radius in edge detection, play around whit the radius slider, shift edge contrast and decontaminate color, those are the keys to get a clean and smooth selection.
