How to make a glass wall mounted plaque in PS

Wondering if anyone has a tutorial or has seen one on how to make this sort of glass wall mounted plaque:

Not got the concentration span to do a tutorial but maybe jsut looking at a psd will be enough to get you started it is not very good and needs to be worked on more but I cant stare at my screen any longer. (I am sick) The PSD will get you this far the rest I am sure you can work out

You will need to change bevel options and sharpen the edges
