I see a combination of Photoshop and illustrator, but saying that it does look a lot like an inked drawing also.

This is the work of Amandeep Singh out of London (Inkquisitve Illustration).

It's hand painted in traditional mediums (mostly watercolor) which can be seen on his MANY tube videos.

To create an image similar to this will require that you have at least an intermediate to advanced understanding of both Photoshop and Illustrator.

What is your current knowledge level of both?

To create such picture you need to have texture of the paper as background, photo of your character, it should be drawn in vector using Illustrator, burned paper, burnt spots textures or brushes, watercolor spot, blots (there is a set of paths in Photoshop) vector or photo of palms. Matches and hearts draw manually in Photoshop or Illustrator. Try to use photo or vector samples from cliparts or google image search if you're a newbie.
