look up low poly art this is pretty much what this image is I have a few tutorials in magazines so I am sure there are plenty online

Hoogle's reply is spot on, but if you want to stay within Photoshop, you can sorta-kinda fake it (lots of further tweaking required) by applying the "cutout" filter ("filter/artistic/cutout").
