Smoothing out a pixelated image

Say I have an image that is sort of pixilated and rough, what is the best way to smooth it out so that it looks a bit better?
 
Your best bet is to get a plug-in like Topaz de-noise, otherwise it is a case of zooming in and getting the blur tool and colour mixing brush to clean the noise and blockyness by hand, while trying not to lose any detail.
 
try the Reduce noise filter.
duplicate the image layer run the Reduce Nose filter with strength at full and other setting at 0.
then change the blending mode to "Darken" or "Lighten"
but it depends on the image
 
I use a program called infranview. It is relatively easy to clean up pictures when using it, and it is very easy and straightforward to learn.
