Specific Transparent background and light tweak

Hello

Could you help me out with removing the background from this wallpaper and make it transparent? The saliva (marked with purple circles) is hard to edit and time consuming to be included so please don't mind it. It can be removed as a part of background and I think it would look a lot better without it.

Also the upper teeth and top of the head (marked with green circles) have some bad lightning and are barely visible. Can you make it more darker or sharper in this areas?

Could anyone help me with removing the background? I've tried to do it myself manual, but with my shaking hand this doesn't look good. Unfortunately I don't know any advanced options to remove it, just with eraser brush
