Using the rulers

I'm having trouble printing images that I want to be a specific size. I use the rulers to create the image the exact size that I want it, but it will print smaller or bigger, apparently at whim.

The printer is not set to scale to fit media, so it seems like if I create something 35mm wide in photoshop that it would then print 35mm wide in real life. What am I missing?
 
You can change the size of an image to an exact size with Image|Image Size or Ctrl+Alt+I on a PC Cmd+Opt+I on a Mac.
Then File|Print or Ctrl+P (Cmd+P on a Mac)

Set Scaled Printing Size to 100%
