QUESTION: How to make image go over signature?

Hello guyz, I'm really newbie at signatures and I wanna figure out how people do this?

How do you let her head go over?

It's not actually "going over" the bounds of the image, there's simply a large part of the image that is transparent.

In Photoshop it looks like this:
